His next battle was against the bubonic plague. He used mandatory reporting case isolation and treatment with serums produced at Manguinhos. He ordered the extermination of the rats that spread the disease. Again he was victorious. But his greatest challenge lay ahead.

The smallpox vaccination controversy. A smallpox epidemic threatened Rio in 1904. A law mandating vaccination existed since 1837. It had never been enforced. Cruz proposed a new law. It mandated vaccination. This sparked the Vaccine Revolt. Rio erupted in violent protests. The government declared a state of siege. Eventually the mandatory vaccination was suspended. But a few years later another massive smallpox outbreak hit Rio. Thousands died. This tragedy vindicated Cruz’s efforts. His methods were proven effective.

In 1909 Cruz stepped down as director general of Public Health. He dedicated himself fully to the Manguinhos Institute which was renamed the Oswaldo Cruz Institute in his honor. He led scientific expeditions. They revealed much about the health and living conditions in the Brazilian interior. His work benefited both human and scientific endeavors.

Osvaldo Cruz’s legacy lives on. His contributions to public health and science remain unmatched. He transformed Brazil’s approach to medicine.
