In 1929, the Berufsgenossenschaft für Gesundheitsdienst und Wohlfahrtspflege was founded. This was the 69th trade association. In the 1930s, some regional associations merged. They created larger national organizations.

After World War II, West Germany maintained the Berufsgenossenschaften. The Soviet occupation zone dissolved them. Some were re-established in West Germany. In the 1950s, self-governance was reorganized. The assembly now included employer and employee representatives.

The Berufsgenossenschaften’s responsibilities grew. They focused on accident prevention. They introduced detailed safety regulations. They also handled non-insurance tasks. This included disbursing child benefits in 1954.

After German reunification in 1990, challenges arose. Integrating East German systems strained finances. This led to criticism of the Berufsgenossenschaften. Some found the system bureaucratic and expensive.

In response, reforms were initiated. The aim was to modernize accident insurance. The number of Berufsgenossenschaften was reduced. Mergers created larger more efficient entities. By 2011, there were nine trade associations.

The European Court of Justice upheld the Berufsgenossenschaften’s monopoly. It stated that compulsory membership did not violate EU law. The court recognized their role in social security.
