On a crisp March day in 1899, the “fast aid” era dawned in St. Petersburg with the inauguration of the Mashina skoroy pomoschi. This marked the beginning of organized emergency medical services in Russia, a pioneering step initiated by surgeon N.A. Velyaminov. The Mashina skoroy pomoschi, literally “fast aid machine” or ambulance station, wasn’t just a building. It was a symbol of modernization and a shift in public perception of healthcare. It demonstrated a commitment to providing rapid medical assistance to those in need. In the 15th and 16th centuries, Russia saw the rise of “God’s people”, individuals who offered charitable aid, including rudimentary medical care. Often impoverished or afflicted themselves, these individuals provided what comfort they could to those even less fortunate. This system, while noble in intention, lacked the structure and training of a dedicated medical service. By the 17th century, designated individuals were appointed to specific city streets to provide aid. These individuals, chosen by Tsar Fedor Etischev, resembled early paramedics, patrolling their designated areas. Their role was primarily to transport the sick and injured to hospitals or provide basic care on-site. The advent of motorized transport in the late 19th century transformed emergency services. In 1897, the first ambulance station, the Mashina skoroy pomoschi, appeared in Warsaw. Similar stations soon followed in Kiev, Odessa, and Riga. The need for a more effective emergency system in bustling cities became evident. Horse-drawn carriages and early motorized ambulances began navigating the crowded streets of St. Petersburg. By 1909, the Mashina skoroy pomoschi in St. Petersburg had evolved into a Central Station, overseeing and coordinating services across the city. This centralized system allowed for a more efficient allocation of resources and a faster response to emergencies. The Central Station also took on the role of training paramedics and ensuring consistent standards of care across all stations. The expansion of the Mashina skoroy pomoschi continued. By 1905, a sixth station was added, specifically dedicated to the First Aid Department. In 1912, a group of 50 doctors advocated for the formal recognition of the Station, highlighting its crucial role in the city’s health infrastructure. The dedicated doctors and staff of the Mashina skoroy pomoschi played a vital role in shaping emergency services. Their contributions, often overlooked, laid the foundation for the modern emergency medical system.
